A free magnetic pouch for your refrigerator holding the medical information first responders need in an emergency.

The File of Life card provided by Chesterfield Township Fire Department in cooperation with Henry Ford Health System

In partnership with Henry Ford Health System, the Chesterfield Township Fire Department offers residents the File of Life completely free of charge.

It is a small, index-card sized magnetic pouch with a fold-out information card, designed to sit on your refrigerator door where first responders are trained to look.

Step by Step

  1. Pick one up

    Stop by fire department headquarters at 33991 23 Mile Road, or ask any Chesterfield fire crew you meet.

  2. Fill in the card

    Record your medical and personal information on the fold-out card.

  3. Put it on the refrigerator

    Keep the pouch on your refrigerator door so responders can find it.

  4. Keep it current

    Update the card whenever a medication, condition, doctor or emergency contact changes, and date it each time.

Ways to Get This Done

What goes on the card

Your name and date of birth, medical conditions, current medications and doses, allergies, recent surgeries, your doctors, your insurance, and who to call in an emergency.

One for everyone in the home

Each member of the household should have their own card. Ask for as many as your family needs.

Why the refrigerator

Every home has one, and first responders are trained to look on the refrigerator door for a File of Life when a patient cannot speak for themselves.

Also available: child seat and vessel ID stickers

The Records Department at Station 3 also hands out child car seat identification stickers and vessel ID stickers for kayaks, canoes and personal watercraft. When you can't speak for them, they speak for you.

Does it cost anything?

No. The File of Life is free to Chesterfield Township residents, provided in partnership with Henry Ford Health.

Where do I pick one up?

At the Records Department in Fire Station 3, 33991 23 Mile Road, Monday to Friday from 8:00 AM to 3:00 PM, or from any Chesterfield fire crew.

Should I keep my information private?

The card stays in your home, on your refrigerator. It is only read by responders who are treating you in an emergency.
