Forms and DocumentsBusinesses holding hazardous chemicals at or above reportable quantities must file an HMIS with the fire department for firefighter safety.
The Hazardous Material Inventory Statement lists the hazardous products held on your site during the calendar year. Reporting is required under MIOSHA Section 14i, the Michigan Fire Prevention Code and Chapter 38 of the Chesterfield Township Code.
The handbook contains everything needed to complete the statement. The statement itself is an Excel spreadsheet with multiple tabs.
What You Will Need
- The completed HMIS spreadsheet covering the calendar year
- An electronic floor plan of the facility
- A current emergency contact form
- All three within 10 business days, unless another deadline is agreed with the fire inspector
